OK, you have to run mtxrun with LuaTeX:

   luatex --luaonly <path-to>/mtxrun.lua --generate

and then everything works. So I guess it *is* just mtxrun, but has to be 
set up properly. Presumably this does happen 'usually', so it must be 
missing from the minimised arrangement we use.
