There is no requirement for .dtx and .ins files.
a) The sources are present, including the source for
the manual.

If you want to exclude any package that
possibly conflicts with another, then
TeX Live would be quite empty, apart
from some text documentation files.
b) License is LPPL. Also no reason
for excluding.
c) Karl has already stated the real reason.
A package must be useful for a system,
which only uses free software.
